Factor Influencing Outcome in Cultivated Limbal Epithelial Tranplantation for Chronic Cicatricial Ocular urface Diorder Original Reearch Article

Purpoe To analyze factor influencing clinical outcome in cultivated limbal epithelial tranplantation (CLET). Deign Retropective, obervational cae erie. Method Twenty-even eye of 27 patient that had CLET for evere chronic cicatricial ocular urface diorder were tudied. Two different cultivation method were ued to prepare epithelial heet. Method 1 ued the explant technique and neither feeder cell nor air-lifting were ued. In Method 2, cell upenion technique and 3T3 feeder cell were ued, and air-lifting wa applied after cultivated cell became confluent. Clinical outcome including corneal urface epithelialization and incidence of potoperative complication were tudied. The relationhip between the clinical outcome and type of cultivation method, original dieae, tear function, or preoperative ocular urface tatu wa alo tudied. Reult Both cultivation method produced tranplantable epithelial heet with corneal phenotype. With a mean follow-up period of 127 week, corneal epithelialization wa achieved in 16 eye (59.3%). Eye that had CLET with Method 1 uffered more evere potoperative complication uch a infection, ulceration, and perforation (P = .053). Eye with teven-Johnon yndrome (J) had poor final corneal epithelialization compared with other dieae (P = .034). CLET wa more ucceful when performed to conjunctivalized cornea compared with eye with peritent epithelial defect or with dermalized cornea. Concluion CLET offer new treatment modalitie to chronic cicatricial ocular urface diorder with moderate ucce rate. The treatment i feaible for eye with non–immune-mediated diorder with table ocular urface condition. Epithelial heet with better tructural integrity eem to be uperior to obtain early potoperative epithelialization and to avoid eriou potoperative complication.
