Abstract :
Purpoe
To invetigate Acanthamoeba keratiti (AK) rik factor. Diagnoi of AK, a rare but eriou corneal infection, ha recently increaed ignificantly at the Univerity of Illinoi at Chicago (UIC) Cornea ervice.
Deign
Retropective cae-control tudy.
Method
etting: Univerity, tertiary care hopital. patient: Fifty-five AK cae with contact len ue were diagnoed between May 1, 2003 and eptember 15, 2006. Clinic-matched control with contact len ue were recruited. ubject completed urvey targeting len hygiene, contact len olution ue, and water expoure. main outcome meaure: Acanthamoeba keratiti.
Reult
Thirty-nine (73.6%) cae and 113 (65.3%) control participated; 38 cae had complete contact len data. Thirty-five of 38 cae (92.1%) and 47 of 100 control (47.0%) ued oft lene. Analyi wa performed on 30 cae and 39 control with matched pair with oft len ue. Excluive ue of Advance Medical Optic (AMO) Complete MoiturePlu Multi-Purpoe olution wa independently aociated with AK in multivariable analyi (55.2% v 10.5%; odd ratio [OR], 16.67; 95% confidence interval [CI] 2.11 to 162.63; P = .008). However, 38.8% of cae reported no ue of AMO Complete MoiturePlu Multi-Purpoe olution either alone or in combination with other olution. Although not tatitically ignificant, additional hygiene-related variable (olution “reue,” lack of “rubbing,” and howering with lene) ugget a pattern of rik.
Concluion
AMO Complete MoiturePlu Multi-Purpoe olution ue i independently aociated with AK among oft contact len uer. However, it doe not explain all cae, uggeting additional factor. Further reearch into environmental rik factor and hygiene practice i warranted, epecially conidering thi i the econd outbreak of an atypical, contact len-related infection.
