Abstract :
Purpoe To evaluate the long-term outcome of keratoprothei a an alternative urgical procedure in the management of aniridic keratopathy. Deign Retropective, multicenter cae erie. Method Fifteen adult patient (16 eye) with aniridia who were deemed at high rik for regular donor corneal tranplantation underwent a Boton type I keratoprothei procedure for viual rehabilitation. Device retention rate, preoperative and potoperative viual acuity, and intraoperative and potoperative complication were tudied. Reult The mean age of the patient wa 45 (median 50) year. ix of the patient were female. Eleven patient had previouly undergone donor corneal tranplantation (average two graft), with failure. In 10 patient, the keratoprothei urgery wa combined with other procedure uch a cataract extraction, tube hunt implantation, vitrectomy, and intraocular len removal. No intraoperative complication were encountered. The follow-up ranged from two month to 85 month, with a median of 17 month. All device remained in itu throughout the entire follow-up period. The viual acuity improved in all but one patient from a median of counting finger (light perception to 20/300) to 20/200 (hand motion to 20/60). Comorbid preoperative condition particularly optic nerve and foveal hypoplaia limited the final potoperative viion. No endophthalmiti or extruion of the device occurred. One device required repair procedure without neceitating a removal. Concluion Keratoprothei offer ignificant viion benefit in thi patient group.
