Abstract :
Purpoe
To evaluate the intercurrent factor for the development of open-angle glaucoma (OAG) in ocular hypertenive patient who were enrolled in the European Glaucoma Prevention tudy (EGP).
Deign
Randomized, double maked, controlled clinical trial.
Method
etting: Multicenter. tudy population: A total of 1,077 patient fulfilled a erie of incluion criteria, including intraocular preure (IOP) 22 to 29 mm Hg, normal and reliable viual field (VF) and normal optic dik. intervention: Treatment with dorzolamide or placebo. main outcome meaure: Glaucoma-related VF or optic dik change. Clinical data were collected every ix month during a five-year follow-up. Proportional hazard model were ued to identify the factor that during follow-up (intercurrent factor) were aociated with the development of OAG.
Reult
In multivariate analye, adjuting for treatment arm and baeline predictive factor, mean follow-up IOP reduction (hazard ratio [HR] 0.89, 95% confidence interval [CI] 0.80 to 0.98), mean follow-up IOP (HR 1.12, 95% CI 1.03 to 1.22), area under the curve of IOP (mm Hg per year) (HR 1.09, 95% CI 1.06 to 1.12), dik hemorrhage (HR 1.97, 95% CI 1.21 to 3.22), and ue of ytemic diuretic (HR 2.41, 95% CI 1.12 to 5.19) were aociated with the development of OAG. Baeline central corneal thickne, vertical cup/dik ratio, vertical cup/dik ratio aymmetry, and pattern tandard deviation remained tatitically ignificant.
Concluion
Thee reult ugget the need for future invetigation to better elucidate the role of ytemic diuretic in the development of OAG, becaue IOP and dik hemorrhage have already been hown to be important intercurrent factor in the Ocular Hypertenion Treatment tudy (OHT) and Early Manifet Glaucoma Trial (EMGT).
