Abstract :
Purpoe To etablih the neceity for an early follow-up examination after an initial funducopic examination with negative reult for patient with acute, ymptomatic poterior vitreou detachment (PVD). Deign Retropective cae-control tudy and meta-analyi. Method Record were reviewed of patient eeking treatment over a 4.5-year period who were diagnoed with an acute, ymptomatic PVD. A MEDLINE earch to identify all publihed obervational cae tudie reporting vitreoretinal pathologic feature after acute, ymptomatic PVD. Reult The incidence of retinal tear in eye with a ymptomatic PVD wa 8.2%. The overall rate of retinal break in the meta-analyi portion of the tudy wa 21.7%. In total, 1.8% of patient had retinal tear that were not een on initial examination. Of the 29 patient with delayed-onet retinal break, 24 (82.8%) had at leat one of the following: vitreou hemorrhage at initial examination, hemorrhage in the peripheral retina at initial examination, or new ymptom. Concluion If the reult of an initial examination of a patient with an acute, ymptomatic PVD are negative for retinal tear, the neceity of early follow-up may be bet determined by the preence of pigmented cell in the vitreou, vitreou hemorrhage, or retinal hemorrhage. Mot patient with ymptomatic PVD may not need an early follow-up examination.
