Abstract :
Purpoe Reliable information i needed to counel parent of children with congenital toxoplamoi regarding the long-term rik of viual impairment reulting from ocular toxoplamoi. Deign Propective cohort tudy of children with congenital toxoplamoi identified by prenatal or neonatal creening. Method After three year of age, ophthalmologit reported the ite of retinochoroidal leion and viual acuity and parent reported viual impairment. An ophthalmologit predicted the child’ viion baed on the lat retinal diagram. election biae were minimized by propective enrollment and data collection, high rate of follow-up, and excluion of referred cae. Reult Two hundred and eighty-one of 284 infected children who underwent ophthalmic examination were followed up to a median age of 4.8 year. One in ix children (49/281; 17%) had at leat one retinochoroidal leion, two-third of whom (32/49; 65%) had a leion at the poterior pole. In children with retinochoroiditi who had viual acuity meaured after 3 year of age, 94% (31/33) had normal viion in the bet eye (6/12 nellen or better), a did 91% of thoe with a poterior pole leion (21/23). Analye baed on affected eye howed that 42% (29/69) had a poterior pole leion, of which jut more than half (15/29, 52%) had normal viion, a did 84% (16/19) of eye with a peripheral leion alone. Viion predicted by the ophthalmologit wa moderately enitive (59%) but overetimated impairment aociated with poterior pole leion. Of 44 children with information on acuity, four (9%) had bilateral viual impairment wore than 6/12 nellen. Concluion evere bilateral impairment occurred in 9% of children with congenital toxoplamic retinochoroiditi. Half the children with a poterior pole leion and one in ix of thoe with peripheral leion alone were viually impaired in the affected eye.
