Abstract :
Purpoe To etablih the caue of uveiti in immunocompromied patient and to determine the contribution of polymerae chain reaction (PCR) and Goldmann-Witmer coefficient (GWC) analyi of aqueou humor in patient with an infectiou etiology. Deign Retropective cae erie of 56 conecutive immunocompromied patient with uveiti. Method All patient underwent full ophthalmologic examination and laboratory blood analyi for uveiti. Aqueou humor analye were performed uing PCR and GWC for cytomegaloviru (CMV), herpe implex viru (HV), varicella zoter viru (VZV), and Toxoplama gondii. Reult Of 56 immunocompromied patient, 43 (77%), all poterior and panuveiti, had intraocular infection. Twenty-one (49%) had CMV, three (7%) had VZV, 11 (26%) had T. gondii, ix (14%) had Treponema pallidum, and one (2%) each had Apergillu and Candida. In AID patient, CMV wa the mot common caue. A trong correlation between AID and ocular yphili wa alo oberved (P = .007). In nonAID immunocompromied patient, T. gondii wa mot frequently detected. Twenty-even patient were examined by both PCR and GWC; five (18.5%) were poitive by both aay, 15 (55.5%) were poitive by PCR alone and even (26%) by GWC alone. Viral infection were detected by PCR in 16 of 17 (94%) cae; T. gondii in four of 10 (40%) patient. Uing GWC, a viral infection wa diagnoed in three of 17 (18%) and T. gondii in nine of 10 (90%) cae. Concluion In immunocompromied patient, PCR i uperior in diagnoing viral infection. Analyi of intraocular antibody production played a deciive role in the diagnoi of ocular toxoplamoi.
