Abstract :
Purpoe To ae a repone to infliximab therapy in childhood uveiti. Deign Retropective cae erie. Method We reviewed the coure of 16 children with noninfectiou uveiti treated with infliximab at an academic medical center. Outcome meaure included incidence of uveiti recurrence, proportion of patient achieving zero or two-tep decline in ocular inflammation, viual acuity, and proportion dicontinuing topical glucocorticoid at zero, three, ix, nine, and 12 month of therapy. Reult Of ixteen children (29 affected eye) with median age 11 year, ix had aociated extraocular inflammatory condition. Fifteen of 16 were treated with concomitant methotrexate. Median follow-up wa 26 month and median maintenance infliximab doe wa 8.2 mg/kg. The median interval between infliximab infuion wa 5.6 week. At one year, 64% achieved zero ocular inflammation, and 79% had zero inflammation or a two-tep decline in inflammation. Topical glucocorticoid were dicontinued in 69%, and 58% remained free of uveiti recurrence at one year. Viual acuity remained table. Infliximab wa dicontinued in two children, one becaue of inefficacy and the other becaue of parental concern about potential ide effect. No advere event occurred. Concluion ixteen children with chronic, noninfectiou uveiti tolerated chronic methotrexate and infliximab therapy. Viual acuity remained table, control of ocular inflammation improved, and reliance on topical glucocorticoid decreaed. High infliximab doe and frequent doing interval were neceary to control uveiti.
