Characteritic of Untreated AID-related Cytomegaloviru Retiniti. II. Finding in the Era of Highly Active Antiretroviral Therapy (1997 to 2000) Original Reearch Article

Purpoe To decribe hot characteritic (ue of highly active antiretroviral therapy [HAART]; CD4+ T-lymphocyte count; HIV ribonucleic acid [RNA] blood level) of people who were diagnoed with AID-related cytomegaloviru (CMV) retiniti after HAART became available and to invetigate effect of HAART on ophthalmic finding. Deign Retropective, obervational cae erie. Method We collected demographic, medical, laboratory, and ophthalmic data for all patient with AID and newly diagnoed, untreated CMV retiniti from January 1997 through December 2000 at 10 ite in Lo Angele and Orange Countie, California. Reult The proportion of Hipanic and African-American patient were equivalent to or greater than their prevalence in the AID and general population of Lo Angele County. Mot patient (n = 80; 63.5%) were known to be receiving HAART at the time of CMV retiniti diagnoi; only 22 patient (17.5%) were HAART-naïve. Median CD4+ T-lymphocyte count wa 15 cell/μl and median HIV RNA blood level wa 103,000 copie/ml for all patient, but in 10 patient, CMV retiniti developed depite good immunologic and virologic repone to HAART. When compared with HAART-naïve patient, HAART-failure patient with CMV retiniti had more aymptomatic dieae (P = .073), better viual acuity in the better eye (P = .003), more bilateral dieae (P = .007), le zone 1 involvement (P = .042), and lower leion border opacity core (P = .054). Concluion Mot patient with AID and newly diagnoed CMV retiniti in an urban etting are HAART-experienced. HAART may influence characteritic of new CMV retiniti leion at preentation, depite laboratory evidence of treatment failure, poibly becaue of reidual CMV-pecific immunity.
