Abstract :
Purpoe
To demontrate technique of iri recontruction (pupilloplaty) at the time of penetrating keratoplaty (PKP) for a variety of pupil abnormalitie, and to evaluate the effect of pupilloplaty on graft urvival and viual outcome.
Deign
Cae-control tudy.
Method
Thirty-five cae (combined PKP and pupilloplaty) and 70 control (PKP alone) were included. Variou pupil abnormalitie and the urgical technique ued to correct them were reviewed and categorized. The Chi-quare and tudent t tet were ued to compare the baeline rik factor and demographic variable between treatment and control group. The known rik factor for corneal graft urvival were analyzed by Kaplan–Meier and Cox regreion method.
Reult
Twenty men and 15 women in the age range, 27 to 77 year underwent combined iri recontruction and PKP with peudophakic/aphakic bullou keratopathy being the mot common indication. The mean follow-up duration wa 39.12 month and eight (22.9%) graft failed during follow-up. Iri recontruction during PKP ha a ignificant poitive effect on long-term graft urvival (hazard ratio 2.52; 95% CI, 1.14 to 5.57; P = .022). Patient with pupilloplaty alo tended to have a better mean potoperative viual acuity compared to the control group (P = .40).
Concluion
Iri recontruction i a relatively imple procedure that hould be performed in eye with ubtantial pupil abnormalitie at the time of PKP. Pupilloplaty ignificantly improve graft urvival, render the pupil cometically more acceptable, and may improve viual outcome.
