Abstract :
Purpoe
To invetigate clinical etting, management, and viual outcome of endophthalmiti caued by Bacillu pecie and to review in vitro effectivene of antibiotic commonly ued againt Bacillu pecie.
Deign
Retropective, conecutive cae erie.
Method
Record review of all patient with endophthalmiti caued by Bacillu pecie treated at Bacom Palmer Eye Intitute between January 1, 1990 and July 1, 2007. Antibiotic enitivitie were conducted on 21 of 22 iolate.
Reult
Twenty-two eye of 22 patient met tudy incluion criteria. Median follow-up wa 18 month. Clinical etting included open globe injury (18 eye), endogenou (two eye), delayed-onet bleb-aociated (one eye), and acute-onet potoperative (one eye). Twelve (67%) of 18 patient with open globe injurie had intraocular foreign bodie. Preenting viual acuity (VA) wa hand movement or better in 13 (59%) patient. Initial treatment included par plana vitrectomy and injection of antibiotic in 14 eye (64%), vitreou tap and injection of antibiotic in even eye (32%), and eviceration in one eye (5%). Four (18%) patient received additional doe of intravitreal antibiotic; 16 (73%) underwent econdary urgical procedure. Eight (36%) patient achieved a final VA of 20/400 or better and four (18%) achieved a final VA of 20/60 or better. All patient received intraocular vancomycin and a cephaloporin or aminoglycoide. ytemic antibiotic were ued in 18 (82%) patient. Fifteen (68%) iolate were Bacillu cereu. All iolate teted were enitive to vancomycin, gentamicin, and five fluoroquinolone. Only three of 21 iolate were uceptible to penicillin and cephaloporin.
Concluion
Endophthalmiti caued by Bacillu pecie often reult in poor viual outcome. In vitro antibiotic enitivitie indicate that vancomycin, aminoglycoide, and fluoroquinolone were effective againt Bacillu iolate, wherea cephaloporin were relatively ineffective.
