Abstract :
Purpoe To determine the characteritic of patient with primary ocular adnexal mucoa-aociated lymphoid tiue (MALT) lymphoma aociated with polyclonal hypergammaglobulinemia. Deign Cae erie tudy. Method Among 81 Japanee patient with primary ocular adnexal MALT lymphoma, even patient (9%) were diagnoed with polyclonal hypergammaglobulinemia. Patient clinical data included a hitory of autoimmune dieae and diemination. Peripheral blood collected from all patient wa analyzed for erum level of rheumatoid factor, oluble interleukin-2 receptor (IL-2R), and immunoglobulin at the time of diagnoi and after each treatment. Reult even patient with polyclonal hypergammaglobulinemia had elevated erum level of rheumatoid factor, IL-2R, immunoglobulin G (IgG), and immunoglobulin E (IgE) at the time of diagnoi. One patient had jogren yndrome. ix patient (86%) had a diemination of the MALT lymphoma or lymphadenopathy at the time of diagnoi. Hitopathologic examination of the patient with lymphadenopathy revealed not only MALT lymphoma but alo econdary follicle. None of the even patient howed improvement in erum level of IgG, rheumatoid factor, or IL-2R in pite of complete regreion of the ocular leion after radiotherapy. After adminitration of cyclophophamide/doxorubicin/vincritine/prednione and/or rituximab to three patient, all three howed improved erum level of IgG, rheumatoid factor, and IL-2R. Concluion Patient with ocular adnexal MALT lymphoma and polyclonal hypergammaglobulinemia have elevated erum level of rheumatoid factor, IL-2R, and IgE, and high diemination or lymphadenopathy. Thee unique characteritic may correlate with the ytemic immunologic imbalance.
