Abstract :
Purpoe To evaluate the interoberver and intraoberver reliability of detecting early and late age-related macular degeneration (AMD) uing a nonmydriatic digital camera in two ditinct group of older people. Deign Propective tudy. Method The two group conited of a erie of patient older than 70 year hopitalized in a geriatric unit and a younger erie of people older than 55 year. In both group, nonmydriatic color fundu photograph were obtained and graded independently by two ophthalmologit (V.L. and M..). No ophthalmic examination wa performed. Main outcome meaure were frequencie of early and late AMD and interoberver and intraoberver agreement. Reult Among 233 patient in group 1 (mean age, 84.6 year), only 119 patient (51%) could undergo photography becaue of aociated multiple morbiditie. Mean age of group 2 wa 63.8 year. In group 1, 35 (14.5%) of 238 picture were ungradable. In erie 2, 65 (9.1%) of 716 picture were ungradable. Frequencie of early and late AMD were 30.3% and 5.9% v 12.6% and 2.6% in erie 1 and 2, repectively. Interoberver and intraoberver agreement wa good or excellent (κ > 0.6) in both group. Concluion In the entire geriatric cohort, 43% of the patient had gradable picture allowing a diagnoi. Thee patient would otherwie have had no acce to any form of funducopy. In the younger population, nonmydriatic picture permitted a diagnoi in 90% of the individual. Detection of AMD with a nonmydriatic digital camera may lead to large-cale creening and pecific management.
