Abstract :
TOMHS is an NHLBI supported study which examined lifestyle and lifestyle plus active drug therapies for treating Stage I diastolic hypertension (DBP 90-59 mmHg). The trial was carried out over 4-6 years in 902 men and women, ages 45-69, initially free of evidence of clinical CHD. Participants were randomized to one of six groups: 1) placebo; 2) beta blocker (acebutolol); 3) calcium blocker (amlodipine); 4) diuretic (chlorthalidone); 5) alpha blocker (doxazosin); or 6) ACE inhibitor (enalapril). All groups received lifestyle intervention. Measures were carried out on BP, side effects, quality of life, biochemistries, lipoproteins, resting and ambulatory ECGs, m-mode echocardiograms and cardiovascular events. Follow-up ranged from 4 to 5.5 years, average follow-up was 4.4 years. The study group was 38% female (n-34.5), 62% male (n-55.7), 20% were African American. Significant lifestyle changes were achieved in all groups. Greater weight loss was associated with favorable changes in BP, lipids, and quality of life. Updated results will be presented on side effects, fasting, serum insulin, quality of life, and other study endpoints.
