Many costs of damage to people, equipments and the environments around the world are due to industrial activities that are often preventable by risk assessment and control measures. The purpose of this descriptive study that was carried out in Kermanshah refinery in 2014 was evaluate the risk of Pentane, Hexane and Pentane storage unit by ETBA. For this purpose, the matrix provided in the MIL-STD 882E standard was designed based on the qualitative classification of severity and risk of occurrence and risk-taking criteria. In this study generally 74 hazards and hazardous energies were identified. Of these 51 (%68.91) undesirable, 21 (%28.37) unacceptable and 2 (%2.70) were acceptable after corrective actions. The highest frequency of hazardous energy 20 (%27.02) were related to electrical energy and the lowest related to energy of the earth 2 (%2.70). Due to the special and dangerous conditions of such environments in the event of an accident, damage to persons and equipments are not unexpected. Among the most important control methods, proper equipments, maintenance and periodical inspections can be mentioned
