Root Cause Failure Analysis for Railway Lifting Jacks

Study was performed to identify the most probable cause of the railway lifting jacks’ incidents in Raja Rail Transportation Company in recent years. The cause of wagon lifting jack failure should be analyzed based on the failure of parts and sub-assemblies. It was decided that the best method to adopt for this investigation was to use Fishbone (Ishikawa) Technique. The information was gathered through an interview with 6 maintenance and repair experts in the field of rail transport. The investigation revealed that the strategy of aggregation of contracts of machinery related to repair and maintenance of wagons- includes wagon lifting jack, was not a suitable solution for this and has not had the optimum effectiveness due to lack of sufficient expert survey / prior studies.
