    Modeling Predictors of Brain Stroke in the Presence of Competing Risk:A Frailty Accelerated Failure Time Approach

    Objectives: Today, due to advances in the diagnosis and treatment of diseases, Brain Stroke (BS) is considered one of the major causes of disability and mortality globally. Therefore, this study is essential to compare the survival rate of BS patients between two causes of death (death due to BS and death due to other reasons). Methods: A total of 332 patients were entered in the ten-year follow-up study (June 2008 up to June 2018). Their event status due to BS or other causes (such as heart disease, etc.) was assessed. The analysis was performed with an accelerator failure time approach of survival analysis with competing risks with frailty parameterization. Results: The results showed, death due to BS age (59-68 years: time ratio [TR] =0.13, 90% CI = [0.06-0.29]), (69-75 years: TR= 0.01, 90% CI=[ 0.01-0.04]), (76+ years: TR= 0.01, 90% CI 0.01-0.06]) and Sex (TR= 0.45, 90% CI=[ 0.25-0.82]), Employment status (TR =0.35 90% CI=[ 0.019-0.66]), Having the heart disease (TR= 0.25, 90% CI=[ 0.12-0.44] and hemorrhagic BS (TR= 0.11, 90% CI=[ 0.05-0.24). Age (59-68 years: TR =0.14, 90% CI= [0.06-0.30]= [0.06-0.29]), (69-75 years: TR= 0.14, 90% CI=[ 0.06-0.30]), (76+ years: TR= 0.10, 90% CI=[ 0.04-0.24]), Place of Residence (TR= 1.71, 90% CI =[1.19-2.44]) were identified as specific factors for death due to other causes. Conclusion: According to the findings of this study in evaluating diagnostic factors, it is better to examine the risk factors affecting the cause of death of patients with BS separately. Having specific information about the cause of death of patients can help improve the patient’s quality of care and treatment outcomes and thus reduce costs.
