Abstract :
Pancreatic metastases are relatively common in advanced lung cancers, but metastase-induced acute pancreatitis is very unusual. In this report, we documented a case of pancreatic metastases of small cell lung carcinoma induced acute pancreatitis. A 74-year-old man was admitted to our hospital with severe epigastric pain and nausea for 24 hours. He had been smoking 25 cigarettes a day for 40 years. There was no history of alcohol consumption or cholesystectomy. Pancreatic enzymes and bilirubin levels were elevated. Radial endoscopic ultrasound (EUS) showed multiple pancreatic hypoechoic masses. Thoracal computerized tomography showed 70 mm and 20 mm masses with irregular margins at the mediastinum and left lung hilus, respectively. Biopsies were taken by using linear endoscopic ultraound (EUS) for the mediastinal mass, pancreatic mass and celiac lymphadenopathy at the same procedure. Three biopsy materials were reported as small cell lung carcinoma in histopathologic evaluation. We concluded that when cases of acute pancreatitis in patients with small cell lung carcinoma are encountered, the possibility of metastase-induced acute pancreatitis should be considered as well as that the only presenting symptom could be the acute pancreatitisis and that pancreatic metastases found in these patients, should be treated aggressively.
