Can Henoch-Schönlein Purpura Be Confused with Acute Appendicitis?
Abstract :
Henoch-Schönlein Purpura is the most common form of immune-mediated systemic vasculitis in children, which mainly affects the skin, joints, gastrointestinal tract and kidney. Abdominal pain is the most common symptom of gastrointestinal involvement. Usually, skin manifestations precede gastrointestinal findings but, in some cases skin lesions occur after gastrointestinal manifestations. In this instance, the diagnosis of Henoch-Schönlein Purpura is very difficult when there is no skin lesions. Henoch-Schönlein Purpura should be considered in the differential diagnosis of an acute abdomen in children. This article describes the case of a pediatric patient who was admitted to hospital with acute abdominal pain, diagnosed with acute appendicitis and operated on, but who then observed intramural hemorrhage of small intestines. Because of the palpable purpura that developed a few days later, the patient s diagnosis was confirmed to be Henoch-Schönlein Purpura. We want to emphasize that a painstakingly detailed examination of the skin is also very significant in terms of purpuric lesionsin patients presenting with acute abdominal pain.
