DOUBLE PYRAMIDAL LOBE DETECTED ON THYROID SCINTIGRAPHY: A CASE PRESENTATION
Abstract :
Thyroid gland may show anatomical variation related with its embryonic developmental stages. Among these variations presence of pyramidal lobe is the most common one. Pyramidal lobe of thyroid gland is the embryologic remnant of the caudal part of thyroglossal duct. Pyramidal lobe is commonly observed in clinical thyroid studies and takes an important place for treatment planning in thyroid diseases. Although a single pyramidal lobe is a common finding, it is rare to see double pyramidal lobes. In this patient presentation we are showing double pyramidal lobes evidenced during thyroid scintigraphy in a patient with hyperthyroidism. To the best of our knowledge, double pyramidal lobes shown on a thyroid scintigraphy have not been reported previously. A 18-year-old female with clinical hyperthyroidism had decreased serum TSH level and diffuse hyperplasia with parenchymal heterogeneity on thyroid ultrasonography. In order to evaluate functional status of the thyroid gland, thyroid scintigraphy with Tc-99m pertechnetate was done. Thyroid scintigraphy showed diffusely increased Tc-99m pertechnetate in the thyroid gland. The additional and interesting finding of thyroid scintigraphy was the presence of double pyramidal lobes extending from the superomedial aspects of both right and left lobes. The patient was put on anti-thyroid therapy with the clinical diagnosis of Graves’ disease.
