Clinical and angiographic results of patients underwent coronary endarterectomy
Abstract :
Purpose: The feasibility of coronary endarterectomy in patients having diffuse coronary artery lesions refered to Coronary Artery Bypas Grafting has become under debate due to different outcomes of coronary endarterectomy studies in literature. The aim of this study is to inform about clinical and angiographical results of patients undergoing coronary endarterectomy and to investigate its applicability. Materials and methods: Between January 2005 and December 2010, 40 patients on whom doctors performed coronary endarterectomy who had undergone CABG were included in the study. Preoperative, operative, early postoperative and late postoperative data were evaluated. Results: Ages of the patients ranged from 43 to 77, with an average of 63,6. Exercise testing and coronary angiographies of patients were evaluated in the follow-up period. The first 30 day mortality rate was 7.5% (three patients in 40); after the first month, mortality rates of patients were 5.7% (two patients in 35); the survival rate was 86.84% (33 patients in 38) (in the following 4.79±1.12 years ) The angiographical patency rate of 21 patients, who underwent coronary endarterectomy, was 66,6% in the following 4.21±1.12. Conclusions: In selected patients with diffuse coronary artery disease, coronary endarterectomy can be used as a tool for complete myocardial revascularization. When mortality, survival and graft patency rates which had been achieved in study were taken into account, it was concluded that coronary endarterectomy was still a remarkable tool. Complete revascularisation chance can be achieved in this way in selected patients.
