Beta-lactams and Aminoglycosides in Neonates at the University Hospital of Cocody in Ivory Coast

Abstract :
Aim: In Ivory Coast, neonatal infections are very frequent. However the antibacterial therapy of these diseases is not still the object of a national consensus. Then it seems important to make the current situation of antibacterial therapy in neonates in reference Hospitals like the University Hospital of Cocody. The aim of this study was to analyze the practice of antibacterial care in neonatal infections there with regard to international recommendations, if need be, to make propositions for improvement. Methods: Two hundred and fourteen neonates hospitalized for infection except parasitic or viral disorders were recruited for seven-month transversal study. Hospital practices were observed and detailed antibacterial therapy was noted. Results: Antibacterial therapy was dominated by doubleantibacterial therapy (98.13%). This concerned in first intention Amoxicillin 100 Mg/Kg twice a day or Ceftriaxone 100 Mg/Kg once a day in direct intravenous injection + Netilmicin or Gentamycin 5 Mg/Kg once a day in intramuscular injection; in second intention (sepsis or altered general state): Cefotaxim 50-75 Mg/Kg twice a day in direct intravenous injection + Netilmicin or Gentamycin 5 Mg/Kg once a day in intramuscular injection. Third-generation cephalosporins were more prescribed at forward neonates than at the premature ones (χ2 = 5.58); antibacterial therapy was generally driven hanging three in ten days (90.19%); 77.1% of neonates presented a regularity in therapy administrations. Conclusions: Intramuscular injections of aminoglycosides and irregularities in some therapy administrations must be corrected to promote an optimal pharmacotherapeutics.
