Clinical Parameters Affecting The Prediction of Cancer in TRUS-assisted Prostate Re-biopsy

Purpose: To establish the rebiopsy criteria’s in patients with negative prior prostate biopsy, we performed a randomized prospective study. Materials and Methods: All patients having blood level of PSA= 4 ng/dl and/or positive DRE (digital rectal examination) underwent transrectal ultrasound guided prostate biopsy. Patients were randomized for 8, 10 and 12 zone biopsies. From each zone 2 or 3 biopsy core was taken. After 6 week, all negative biopsy patients having rebiopsy criteria’s were performed 8, 10, 12 and two additional transitional zone biopsy. Rebiopsy criteria’s were; findings of high PIN, resisting High level of blood PSA, increasing PSA velocity 0,75 ng/dl/year. Results: 90 out of 397 patients in first ultrasound guided transrectal biopsies, and 14 out of 101 rebiopsy patient, diagnosed to have of Prostate cancer. The most important cancer predicting criteria’s for re-biopsy were % fPSA, PSA velocity and TZPSAD. Conclusions: In our study, the rebiopsy criteria’s were found for fPSA, PSA velocity, TzPSAD, PSAD and Tz volume.. In this study we conclude that fPSA, PSA velocity and TzPSAD are important determinants of prostate re-biopsy.
