Abstract
The mortality rate of secondary biliary peritonitis is still high despite recent novel advancements in treatment. The principles of management include eliminating the source, reducing the contamination, preventing recurrent infection. Several techniques like etappenlavage introduced the development in reducing the degree of contamination. However the control of sepsis focus determinates the success of treatment. The control of bile leak involves the repair of the leak site and decompression of biliary tree. But severe inflammation makes it impossible to dissect the hepatoduodenal ligament. Intraoperative endoscopic retrograde cholangiopancreatography (ERCP) may be valuable to control the biliary system and to prevent the inadvertent iatrogenic organ injury.
