Abstract :
There is need to more effectively question decisions on equipment reliability within the framework of effects on reliable operation. This is addressed by improving and coordinating methods for classifying and evaluating potential, prospective and actual failure problems. As a means of improving equipment reliability, practical approaches are proposed for effectively coordinating evaluations and formal communication among responsible groups. These principles are applied to predicting failure occurrences; this enhances tactical planning as well as evaluation. Prediction, in turn, is supported by expanded and more sophisticated statistical and narrative data bases; statistical bases are described generically for use by a variety of groups responsible for reliability.
