Abstract :
With the continuous follow-up visits to patients, the aim is not only to promote their life quality and survival time, reduce the family burden and society burden, but also is beneficial to conduct clinical teaching and research activities. We developed a liver transplantation follow-up management system with personalized management applications according to the characteristics of liver transplantation patients and their follow-up management requirements. The system is intended to formulate and implement the personalized follow-up management plans for patients through the establishment of follow-up objects, follow-up medical records and the return visit process by a series of data of patients, for example, input or import data of patients such as the basic information, diagnosis, the course of disease, medication etc. The establishment of follow-up system realized a standardized follow-up management model to patients, improved the working efficiency of follow-up staffs and achieved a digital management model which integrated the clinic, teaching, scientific researching and quality services. In conclusion, it is necessary to take a standardized follow-up visit to liver transplantation patients by using information platform, which help of improving the rate of accurate diagnosis and working efficiency, and it is also beneficial to carry out the scientific researching and teaching.
