Abstract :
A Patient-Centred Medical Home Model has been implemented at Group Health Cooperative. The implementation includes integrated Electronic Medical Records, a team approach to primary care, and an emphasis on accessibility, chronic disease management, and patient education. The result has been decreased medical costs, improved patient satisfaction, increased clinician retention, and decreased emergency room visits and hospitalizations.
