Abstract :
The foundation of the retrospective and predictive analysis in Cognitive Reliability and Error Analysis Method (CREAM) is the search of the event sequence caused by human errors. However, there are no standards for CREAM, especially for the analysis of weapon system operation process. This paper presented an improved CREAM based on HAZOP analysis by which the operation steps can be partitioned explicitly, and the causes and the consequences can be evaluated through the deviations analysis. Based on HAZOP analysis, the event-order diagram that described the accident process caused by human error can be acquired. After analyzing the selected event sequence, the root causes of human errors and the probability interval of event sequence can be obtained. An example of diesel engine startup is discussed, which demonstrated the validity of the method for weapon system safety analysis.
