  • Abstract
    Community care provides seamless help for elders, and it covers many organizations, such as hospitals, day care centers, nursing homes, residences, etc. However, these organizations lack of interoperability is due to the difference of information technology adoption. The nursing home often accepts chronic disease elders, but the quality of care is insufficient to satisfy them, since it always provides basic life care and pays no attention to how information technology can help. In this study, we focus on institutional care. By exploring some classic topics in developing computer-based system based on interviews with typical health care institutions, we find some potential for IT in institutional care. We propose a community care platform architecture to integrate the care institution and large-scale hospital, and expect it can help to form a complete community care regime and to improve the quality of care
