In Last Decade various agile software development methodologies evolved. But still agile implementation is the most critical task in the software development industry. The most probable reason for that is lack of training, communication, planning and incorrect communication of the user stories. There is lot of survey proved the success of the agile in the software industry and their productivity growth. Same survey has shown the failure rate of agile in the industry but when compare to the traditional method of software development agile is most success one in reality. Initially while starting agile software development in practice we don´t feel a much problem, when later moved on to the stages of software development the problem what facing is critical ones. Lot of researches and proposal made on solving those problems but still the problems alive. Here in this paper the study is made on each stage mistakes made by individuals made during agile software development cycle, which is not addressed by other papers and proposed theoretical solution is made by using poka yoke concept.
