Abstract
The topic of incident investigation has been addressed in professional literature for many years. In general, investigation literature can be characterized as reactive activities and theories - recommended practices after an incident has occurred. This presentation uses knowledge of human behavior to develop an effective root cause investigation technique for human errors. The process integrates with modern human error prevention programs, is easily applied, and provides logically connected typical corrective actions.
