    The author discusses fail safe railway signalling systems and their failure. The investigation of `wrong side´ signalling failures is discussed. Examples of reliability and wrong side failure investigations are given. The failures covered include silver and copper ions migration in contacts, solid state interlocking (SSI) problems, and interference from traction systems and track circuits
