Abstract :
This paper reports on a project focussed on developing a tool which aids self management of Diabetes, a long term chronic illness. The Diabetes Online Patient Management (DO-PM) tool was developed to allow patients to record, visualise trends and peaks in glucose measurement and, to send - ahead of an appointment, samples of the measures to the clinic staff or the doctor who provides care for the patient. The advantage this offers the patient is that trends and peaks can be visualized easily and samples from given dates can be examined and sent to the clinician to ascertain whether an appointment is necessary sooner rather than later. The prototype of the system has demonstrated the requirements necessary for enabling such on-line tools and the benefits it affords the patient and the practitioner.
