Abstract :
The care of the amputee becomes much better when health professionals know and evaluate each of these individuals as well. This research aims to develop a tool that facilitates and benefits these professional, especially physiotherapists, proposing an electronic record that was made on the basis of standard physical therapy evaluation forms for amputees. The software, called AMPARE was developed in ASP.NET with AJAX / VB.NET / C #. NET using the PostgreSQL database, and allows access via the Internet. It covers the following: Patient Registration, History, Etiology, Physical Examination, Postural Assessment, Postural Scheme, Stump Evaluation, Reviews March, and the application of the formulation of reports. It is believed that these reports corroborate with the statistical analysis of data acquired in electronic record, facilitate the exchange of information between rehabilitation centers and consequently will impact patient care by improving compliance to their new condition and even their quality of life.
