  • Abstract
    Generally in Brazil, health records, such as patient´s records, are documents in paper form, stored in different locations, which makes access and data integration difficult. Moreover, there is no standardization of terms used between the assessments. However, these evaluations allow to draw a more appropriate course of treatment. In order to provide greater safety and practicality in the assessment and efficacy in the treatment of neurological patients, it was created the SYSNAPSE: physical therapy electronic report for assessing adult neurofuncionnal. This is software developed in PHP, Open Source language and Java Script, Ajax library and Lightbox. It will run on Linux with Tomcat web server, using MySQL or Postgres SQL. The SYSNAPSE allows registering and monitoring neurological adults patients with greater ease and speed of access to patient information. Moreover, health institutions can benefit of statistical data on the prevalence of certain neurological disease.
