تعيين موانع پياده سازي پرونده الكترونيك سلامت از ديدگاه مسئولان واحد فناوري اطلاعات و مديران بيمارستان هاي شهر كرمان

Barriers to Implementing Electronic Health Records from the Perspective of IT Administrators and Hospital Managers in Kerman

مقدمه: با وجود توسعه فناوري اطلاعات به ويژه در حيطه بهداشت و درمان، هنوز فرايند ايجاد و به كارگيري پرونده الكترونيك سلامت دشوار مي باشد. شناسايي موانع اجرايي اين سيستم به رفع آنها و اتخاذ راهكار هاي موثر پياده سازي كمك مي نمايد. هدف اين مطالعه تعيين موانع اجرايي پياده سازي پرونده الكترونيك سلامت در بيمارستان هاي كرمان بود. روش كار: اين مطالعه توصيفي- مقطعي در سال 1394 انجام گرفت. جامعه اين پژوهش شامل مسئولان فناوري اطلاعات و مديران بيمارستان هاي شهركرمان بود و به دليل محدود بودن تعداد افراد نمونه گيري انجام نشد. داده ها از طريق پرسشنامه پژوهشگر ساخته جمع آوري گرديد. روايي آن براساس روش اعتبار محتوا و روايي صوري و پايايي آن ازطريق محاسبه همبستگي دروني تاييد شد (0/93=α). داده ها با استفاده از آمار توصيفي و آزمون T-Test در نرم افزار SPSS19 تجزيه و تحليل شدند. يافته ها: بيشترين ميانگين موانع پياده سازي سيستم، مربوط به محدوديت هاي فني و تخصصي (32) و نگرشي- رفتاري افراد (25/1) و كم ترين آن مربوط به محدوديت هاي استاندارد سازي (9/3) بود. هيچ كدام از محدوديت هاي اجرايي با اطلاعات جمعيت شناختي شركت كنندگان ارتباط معني داري نداشتند(p<0/05). بين محدوديت هاي فني و تخصصي، تغييرات سازماني، هزينه‍اي، نگرشي- رفتاري افراد باپياده سازي اين سيستم در بيمارستان هاي مختلف ارتباط معني داري وجود داشت (0/05=P). نتيجه گيري: محدوديت هاي فني، تخصصي و نگرشي- رفتاري مهم ترين موانع پياده سازي پرونده الكترونيك سلامت مي باشند. به منظور غلبه بر موانع راهكار هايي نظير برنامه ريزي جهت پياده سازي و استفاده از اين سيستم، ايجاد تيم متخصص براي ارزيابي موانع احتمالي، آموزش كليه مديران ارشد و كاربران توصيه مي شود.

Introduction: Despite the diffusion of information technology, especially in the health care domain, the development process and use of Electronic Health Record is still difficult; consequently, it is important to identify and eliminate barriers to implementing this system, and adopt effective implementation approaches. This study therefore aimed to determine the barriers to implementing Electronic Health Records in hospitals in Kerman. Methods: In this descriptive, cross-sectional study, conducted in 2015, all IT administrators and managers of hospitals in Kerman were recruited. Data were collected using a researcher made questionnaire, the content and face validity of which was confirmed and its reliability was calculated using internal consistency (α=0.93). Data were analyzed by SPSS 19 using descriptive statistics and an independent samples T-Test. Results: The highest mean scores of barriers were related to technical and professional barriers (32), and attitude and behavioral barriers (25.1) and the lowest was related to standardization barriers (9.3). None of the barriers were significantly correlated with demographic information (p>0.05). There was a significant correlation between technical and professional, organizational, financial, attitude and behavioral barriers and the implementation of Electronic Health Records in different hospitals (p=0.05). Conclusion: The most important barriers to implementing Electronic Health Records are technical and professional and attitude-behavioral barriers. In order to overcome these barriers the following strategies are recommended: to implement and use Electronic Health Records, establish an expert team evaluating the potential barriers, and educate senior managers, users and providers
