جايگاه و موانع اجرايي طرح پزشك خانواده در نظام سلامت ايران

Status and Barriers to Implementation of Family Physician Program in Iran Health System

مقدمه: طرح پزشك خانواده به عنوان يكي از مهم ترين سياست گذاريهايي است كه در دهه اخير در عرصه نظام سلامت مطرح شده، ازاين رو اهميت و ضرورت مطالعه آن دقيقاً از اهميت و جايگاه سلامتي در توسعه و رفاه جامعه نشات مي گيرد. بدين جهت، هدف پژوهش حاضر، مطالعه جايگاه و روند اجرايي برنامه پزشك خانواده در نظام سلامت ايران است، اينكه از چه پيشينه تاريخي، قانوني و اجرايي برخوردار است؟ و در زمينه اجرا با چه چالشها و موانعي مواجه بوده است؟ روش: پژوهش حاضر در قلمرو پژوهش كيفي است كه در آن نمونه گيري مبتني بر هدف است و براي جمع آوري داده ها از تحليل اسنادي و مصاحبه استفاده شده است. تحليل داده هاي حاصل از مصاحبه نيز از طريق تحليل مضمون بر اساس كدگذاري صورت گرفت. يافته ها: هم زمان با دستيابي به پيشينه تاريخي، مباني قانوني و روند اجرايي برنامه پزشك خانواده، موانع اجرايي برنامه از نظر بستر و زمينه هاي آن تحت عنوان 4 مقوله اصلي شناسايي شد: «قائم به فرد بودن برنامه ها و تاثير تغييرات مديريت و دولت بر برنامه ملي»، «چالش ناشي از نحوه حمايت دولت دهم از پزشك خانواده»، «چالشهاي فرهنگي»، «قانون گريزي، عدم اراده و تعهد سياسي مسئولين ارشد كشوري». نتيجه گيري: پزشك خانواده كه از سال 1381 به تدريج در نظام سلامت مطرح شد، مي بايست برحسب قوانين موجود، چون برنامه هاي 5 ساله توسعه چهارم و پنجم تاكنون در سرتاسر ايران اجرايي مي شد، ولي آنچه در عمل به وقوع پيوست، محدود شدن اجراي آن به روستاها و دو استان فارس و مازندران آن هم به سبك و سياقي متفاوت از خود برنامه پزشك خانواده است كه در اين راستا مطالعه موانع اجرايي آن نمايان ساخت كه بسياري از زمينه هاي اقتصادي، اجتماعي، سياسي و فرهنگي كه لازمه اجراي برنامه بودند، مهيا نبوده، زمينه هايي كه نقش كليدي در موفقيت و شكست هر برنامه اي داشته و در عرصه سياست گذاري ضرورتاً بايد بدانها توجه شود.

Introduction: Family physician program is one of the most important policies that has been proposed in the past decade in the field of health system. Hence the importance and necessity of studying it originate from the importance and the status of health in development and welfare of a society. Therefore، this study aims to investigate the status and the process of the implementation of the Family Physician Program in Iran's health system. This study answers these questions that what is the historical، legal and execution background of family physician program? and what are the challenges and obstacles to implement this program? Method: This research was a qualitative study applying the purposive sampling method . .Data were collected through applying interview and document analysis techniques. The data drawn by interview were analyzed using content analysis، which was based on coding. Results: Concurrent with detecting the historical، legal and execution background of the family physician program، executive barriers were identified based on four categories: “dependency of plans to person and the impact of management and government changes on the national program”، “challenges emanating from the way the tenth government supports family physician program”، “cultural challenges” and “illegality and lack of political determination and commitment of senior officials”. Discussion: Family physician program that has been gradually introduced in the health system since 2002 should have been implemented across Iran، according to the existing law such as the fourth and fifth development plans. However، in practice its implementation was limited to villages and two provinces of Fars and Mazandaran and with different procedures . This study showed that many of the economic، social، political and cultural conditions which had a key role in the success or failure of any program and should be taken into account in policy making were not satisfied.
