A Case of Fifth Metacarpal Neck Fracture

دانستن مكانيزم ضربه و صدمه يكي از اقدامات مهم در كارشناسي هاي پزشكي قانوني است. مكانيزم ايجاد شكستگي گردن متاكارپ، ضربه مستقيم به مشت فشرده شده است. غالبا اين نوع شكستگي از اصابت مشت بسته به جسمي سخت ايجاد مي شود. در اين مطالعه، به معرفي يك مورد از اين شكستگي مي پردازيم. مشخصات بيمار مصدوم مرد ميان سالي بود كه در سال 1397 به دنبال نزاع از نواحي مختلف بدن متالم بوده و به پزشكي قانوني مراجعه كرده بود. صدمات وي علاوه بر چندين خراشيدگي سطحي، شكستگي گردن متاكارپ پنجم دست راست نيز بود. مقام قضايي طي مكاتبه اي از پزشكي قانوني درخواست نمود كه براي احقاق حق طرفين، از آنجايي كه شاكي اذعان نموده صدمات وارده به وسيله كمان اره آهن بر ايجاد شده است، اعلام شود كه با توجه به نوع صدمه، ايجاد آن با چه وسيله اي بوده است. در پاسخ پزشكي قانوني به مقام قضايي مشخص شد كه شكستگي استخوان پنجم كف دست راست ناشي از اصابت كمان اره نيست. نتيجه گيري چنانچه پزشك قانوني در معاينه مراجعان به مكانيزم تروما توجه نداشته باشد به دليل عدم افتراق موارد تروماي ناشي از حادثه از تروماي خودساخته (ساختگي)، ممكن است منجر به تضييع حقوق افراد و سلب اعتماد قربانيان شود و از طرف ديگر موجب سوء استفاده خاطيان گردد. در راستاي تشخيص مكانيزم تروما و نحوه حدوث ضايعه، تعامل با دستگاه قضايي بسيار راه گشا بوده و به هيچ عنوان نبايد از مكاتبه و تعامل با مقام قضايي چشم پوشي كرد.

Often, this type of fracture is due to the impact of the first depending on the hard. In this study, we introduce a case of this fracture. Patient’s Information He was a middle-aged man, who had ache with various areas of the body followed by a quarrel, and came to the forensic medical center in 2018. His injuries included fifth right metacarpal neck fracture in addition to several superficial scratches. The judiciary asked forensic medicine to declare that, inasmuch as the plaintiff acknowledged that the damage had been caused by the iron cutter bow saw, according to the type of injury, with what it was created. Replying to judiciary, the forensic medicine declared that the fifth right metacarpal neck fracture was not due to a bow saw. Conclusion If the forensic medicine does not pay attention to the mechanism of trauma in the examinations, due to the non-differentiation of the traumatic consequences of the incident from a traumatic self-made (fabricated) trauma, it may lead to the violation of the rights of individuals and the loss of confidence of the victims and, on the other hand, it will lead to misuse of the perpetrators. In order to identify the mechanism of trauma and the way, in which the lesion is established, interaction with the judiciary is very promising and correspondence and interaction with judiciary should not be ignored in any way.
