ترسيم نقشه علمي مقالات حوزه تروماي نظامي با استفاده از تحليل هم واژگاني در مدلاين

Scientific Mapping of Military Trauma Papers using Co-Word Analysis in MEDLINE

تروماي نظامي داراي الگوي آسيب هاي چند بعدي است و مديريت مجروحان تروماي جنگي و توسعه دانشهاي مرتبط با آن براي حفظ و ارتقاء آمادگي حوزه طب نظامي ضروري است. اين پژوهش با هدف بررسي روند و ترسيم نقشه علم تروماي نظامي بر اساس مقالات منتشر شده در پايگاه علمي و استنادي مدلاين انجام شد. روش ها پژوهش حاضر از نوع مطالعات كاربردي با رويكرد علم سنجي است كه با استفاده از تحليل هم رخدادي واژگان انجام شد. توصيفگرهاي مقالات مرتبط با جستجوي عبارت «military trauma» در سرعنوان موضوعي «MeSH» ، كه طي سالهاي 2000 تا 2017 در پايگاه علمي و استنادي مدلاين نمايه شده بودند، با استفاده از ابزار جستجوي پاب مد استخراج شدند. تجزيه و تحليل داده ها با استفاده از شاخص شماي وزن دهي TF-IDF و تحليل هم رخدادي واژگان انجام شد. از نرم افزار Word Stat ، N-vivoو VOSviewer براي بصري سازي نقشه و ترسيم نقشه علمي استفاده گرديد. يافته ها رشد توليدات علمي مربوط به حوزه تروماي نظامي، نمايه شده در پايگاه مدلاين در بازه زماني 2000-2017 سير صعودي داشته و با نرخ رشد نسبتا ثابتي بوده است. براساس شاخص TF-IDF، مفاهيم عمومي از قبيل: كهنه سربازان، مديريت مجروحان و مصدومان و دلايل مجروحيت و واژگاني شامل: اختلالات استرسي پس از آسيب، جراحي، افسردگي و شكستگي از پركاربردترين موضوعات اين حوزه به شمار مي روند. نتايج تحليل هم واژگاني در مقالات بيانگر تمركز موضوعات در زمينه هاي PTSD، نوع زخم، مديريت مجروحان، شرايط رزم و تجارب جنگ هايي مانند عراق و افغانستان بوده است. نتيجه گيري ساختار موضوعي توليدات علمي تروماي نظامي در طول زمان تغيير يافته و به صورتي پويا بر اساس مسايل و مشكلات نيروهاي نظامي در حوزه آسيب ها و تجارب جنگ هايي اخير رويكرد مطالعات نيز تحت تاثير قرار گرفته است. نقشه علم تروماي نظامي اطلاعاتي از موضوعات را اصلي در اختيار كاربران و مسئولين سيستمهاي سلامت نظامي قرار مي دهد. باتوجه به تهديد وسيع جنگ هاي نامتقارن و حوادث تروريستي، توسعه دانش تروماي نظامي مرتبط با موضوعات اين نقشه در نظام مراقبت هاي نظامي به كار گرفته شود.

Background and Aim: Military trauma has a ‘multidimensional’ injury pattern and the management of traumatic wounds and the development of related knowledge is essential to maintain and enhance the readiness of military medical personnel. This research was conducted with the aim of investigating the trends and science mapping of military trauma papers in MEDLINE. Methods: This applied research was conducted using a scientometric approach and co-occurrence analysis. The descriptors of papers related to the search for the phrase "military trauma" in the medical subject heading (Mesh), which were indexed between the years 2000 and 2017 at the Medline Science and Citation Center, were extracted using the PubMed search tool. Data analysis was performed using the TF-IDF weighting scheme and co-occurrence analysis. Word Stat, N-vivo and VOS viewer software were used to perform scientific mapping. Results: The growth of scientific research in the field of military trauma indexed on MEDLINE database has a rising trend and the growth rate is relatively stable between the years 2000 and 2017. Based on the TFIDF index, general concepts such as veteran/war veterans, management and casualty wound and words including posttraumatic stress disorder, surgery, depression and fracture are the most frequently cited topics. The results of the co-occurrence analysis in the papers indicate the focus on PTSD, injury type, wound management, combat situations and war experiences such as those in Iraq and Afghanistan. Conclusion: The thematic structure of the scientific production of military trauma has changed over time and has been dynamically influenced by the problems of the military forces in the area of the damage and experiences of recent wars. The military trauma science mapping tool facilitates recognition of this important area and provides intelligence of the main issues to the users and authorities of military health systems. Given the widespread threat of asymmetric wars and terrorist incidents, the development of the knowledge of military trauma related to the subject matter of this map may prove useful in the military care system.
