بررسي فعاليت ‌هاي بخش مدارك پزشكي بيمارستان ‌هاي آموزشي وابسته به دانشگاه علوم پزشكي و خدمات بهداشتي تهران

The assessment of medical record department activities in Tehran Hospitals

تحقيق حاضر يك مطالعه كاربردي - توصيفي است كه در آن كليه بيمارستان‌ هاي آموزشي درماني وابسته به دانشگاه علوم پزشكي و خدمات بهداشتي درماني تهران اعم از عمومي و تخصصي مورد بررسي قرار گرفته است. (كلا 15 بيمارستان) و هدف عمده تعيين وضعيت فعاليت‌ هاي بخش مدارك پزشكي اين بيمارستان‌ها مي باشد. در راستاي نيل به اين هدف كلي اهداف ويژه‌اي بدين شرح نيز مدنظر قرار گرفته است: - تعيين وضعيت بخش مدارك پزشكي از نقطه نظر چگونگي خط مشي‌ها و فعاليت‌هاي واحد پذيرش، واحد مدرك پزشكي، واحد بايگاني، واحد كدگذاري و واحد آمار. - مقايسه يافته‌هاي حاصل با اصول پذيرفته شده جهاني و استانداردهاي داخلي. اطلاعات مورد نياز از طريق پرسشنامه‌اي كه شامل 7 قسمت مجزا بوده و مشتمل بر سوالات مربوط به پذيرش اهم نتايج حاصل بشرح زير مي‌باشد: در اكثر قريب به اتفاق موارد بخش‌هاي مدارك پزشكي بيمارستان‌ها خط مشي و يا دستورالعملي مبني بر نحوه فعاليت‌ها ندارد. در هيچ يك از مراكز مورد بررسي فضاي كافي جهت بخش مدارك تخصيص داده نشده است. در اكثريت قريب به اتفاق موارد اقدامات ارزيابي جهت فعاليت‌هاي واحد مدارك پزشكي انجام نمي‌گيرد. هيچ يك از مراكز به دليل اينكه منابعي در زمينه روزآمد كردن فعاليت‌ها ندارد تغييري در سياستهاي اجرايي نداده‌اند. هيچ نوع سيستم كنترل كننده‌اي جهت مهار نمودن تعداد پرونده‌هاي ناقص توسط پزشكان وجود ندارد. به استثناي يك دوره كلاسهاي بازآموزي گروه مدارك پزشكي كه توسط معاونت پژوهشي وزارت متبوع برگزار شده هيچ نوع فعاليت آموزشي ديگري در زمينه ارتقا كيفيت مدارك پزشكي مراكز صورت نگرفته است. در كليه بيمارستان‌هاي مورد بررسي اطلاعات موجود در پرونده را براي بيمار محرمانه دانسته ولي خط مشي خاصي خاصي در زمينه نحوه در اختيار گذاردن اطلاعات نداشته و در هيچ موردي نيز اجازه بيمار را براي در اختيار گذاردن اطلاعات لازم نمي‌دانند. در هيچكدام از مراكز آموزشي درماني تحت مطالعه هيچ نوع كميته‌اي تشكيل نمي‌گردد. به استثناي يك مركز تخصصي كه داراي كميته مرگ و مير است و مسوول مدارك پزشكي نيز عضوي فعال در آن كميته مي‌باشد

The study included the following objectives: 1- Assessing the condition of the records from the prospect of policy, admission affairs, medical records, filling, coding, and statistics. 2-Comparing the present condition with the accepted national and international standards. Data for this study was collected by employing a questionnaire that included questions related to admission affairs, medical records, filling, coding, statistics, general activities and policy of managing and educational activities. The analysis of data showed the following results: 1 - In most cases, the medical record section in hospitals do no have any policies for their activities. 2- There was insufficient room for medical records in all hospitals. 3- No evaluative steps for activities of medical records were taken. 4- None of tlie centers attempted to modify or update their executive policies due to the lack of sources 5- No controlling system existed that could prevent the filing of incomplete files by physicians. 6- Except a re-training course that was presented by the Deputy of Research of the Ministry of Health and Medical Education to the medical record group, no other educational activities was carried to improve the quality of medical record centers. The medical records are considered confidential in all hospitals, but no specific policy egarding the release of information exist or in no case the permit of patient for releasing information was considered necessary. None of the educational centers in this study forms a committee except a specialized center that has the mortality committee ant the person in charge of medical record is an active member of that committee.
