تحليل يك مورد كودك‌آزاري با رويكرد اخلاقي

Analysis of a Case of Child Abuse with Ethical Approach

زمينه و هدف: خشونت عليه كودكان يكي از چالش‌هاي عمده براي كاركنان مراقبتي است كه سلامت جسمي و رواني كودكان را به مخاطره انداخته و مشكلات عديدي را در آينده براي آنان پديد مي‌آورد. اين مطالعه با هدف تحليل يك مورد كودك‌آزاري با رويكرد اخلاقي در سال 1395 انجام شده است. مواد و روش‌ها: در اين مطالعه موردي، يك مورد واقعي در جلسه ماهانه اخلاق پزشكي، مركز طبي كودكان تهران معرفي گرديده، سپس تحليل اخلاقي بر اساس ملاحظات اخلاقي مطرح‌شده توسط كاركنان درماني و متخصصان حيطه‌هاي مختلف انجام شد. مورد، پسربچه 5 ساله‌اي است كه مورد آزار جسمي قرار گرفته و با شكستگي‌هاي متعدد در نواحي مختلف دست‌ها و همچنين سوختگي دست و پا، توسط پدر و مادرش در شيفت صبح به واحد اورژانس مركز طبي كودكان آورده شد. ملاحظات اخلاقي: ضمن رعايت صداقت و امانت‌داري در گزارش مورد، سعي گرديد، محرمانگي اطلاعات فردي مورد و خانواده وي رعايت گردد. يافته‌ها: اخذ شرح حال كامل پزشكي و اجتماعي، معاينه كامل بدني و بدون پوشش، عدم پنهان‌كاري آزارهاي جنسي كودكان، احترام به كرامت انساني، حقيقت‌گويي، حفظ حريم خصوصي، رازداري و محرمانگي اطلاعات، رفتار احترام‌آميز با خانواده، احترام به استقلال كودك، بي‌طرفي و حمايت اجتماعي از مهم‌ترين ملاحظات اخلاقي مورد اشاره بودند. همچنين كمك به سازگاري خانواده‌ها در جهت پيشگيري از رخدادهاي بعدي كودك‌آزاري، از ديگر يافته‌هاي تحليل حاضر بودند. نتيجه گيري: پيشنهاد مي‌شود تا براساس ملاحظات اخلاقي طرح‌شده، برنامه مدوني در اين رابطه توسط دست‌اندركاران امر طراحي و در اختيار مراكز درماني كل كشور قرار گيرد. همچنين يك گروه متشكل از متخصصين اطفال و اخلاق پزشكي، مددكار اجتماعي، روان‌پرستار و روانپزشك كودك نيازهاي كودكان آزارديده و خانواده‌هاي آنان را ارزيابي نموده و در جهت رفع مشكلات آنان برنامه‌ريزي نمايند

Background and Aim: Violence against children is one of the major challenges for health care providers that has endangered their physical and mental health and creates serious problems for them in the future. This study aimed to analyze a case of child abuse in 2016 with an ethical approach. Materials and Methods: In this case study, a real case was referred to Tehran Children Medical Center in a monthly session of medical ethics, and the ethical analysis was later carried out based on ethical considerations raised by the medical staff and specialists in various fields. The case is a 5-year-old boy who was physically abused and was brought to the emergency department at Children's Medical Center in the morning due to the multiple fractures in different parts of the hands as well as cigarette burn caused by his parents in his hands and feet. Ethical Considerations: While observing honesty in the case report, attempts were made to respect the confidentiality of personal information of the case and his family. Findings: Obtaining complete medical and social history thorough examination of the naked body, lack of secrecy about sexual assault on children, respect for human dignity, honesty, confidentiality and privacy, respectful conduct with family, respect for the child's independence, neutrality and social protection are considered as the most important ethical considerations. Also, helping families adapt in order to prevent further incidents of child abuse is another finding of the analysis. Conclusion: It is suggested, the relevant authorities design a written program based on the ethical considerations raised and submitted it to the medical centers of the country. Also, a group consisted of pediatrics and medical ethics, social workers, psychiatric nurse and psychiatrist should assess needs of abused children and their families and plan to solve their problems.
