همبستگي درماندگي اخلاقي و ديدگاه پرستاران نسبت به رعايت حقوق بيماران در شرق ايران

Moral distress and Nurse’s Attitude toward Patients' Rights

زمينه و هدف: رعايت حقوق بيمار جزء مهم‌ترين مؤلفه‌هاي ارائه مراقبت اخلاقي و كرامت‌مدار بوده و پرستاران ممكن است در دفاع از حقوق بيمارشان در معرض درماندگي اخلاقي قرار گيرند. مطالعه حاضر با هدف تعيين همبستگي بين درماندگي اخلاقي و ديدگاه پرستاران نسبت به رعايت حقوق بيماران صورت گرفته است. مواد و روش‌ها: مطالعه همبستگي حاضر روي 194 نفر از پرستاران شاغل در بخش‌هاي مراقبت ويژه، بيمارستان‌هاي آموزشي وابسته به دانشگاه علوم پزشكي استان خراسان جنوبي در سال 1395 كه به صورت در دسترس انتخاب شدند، صورت گرفت. ابزار جمع‌آوري ‌كننده اطلاعات شامل پرسشنامه درماندگي اخلاقي Corly و پرسشنامه پژوهشگرساخته سنجش ديدگاه پرستاران نسبت به رعايت حقوق بيمار بود. داده‌هاي جمع‌آوري‌شده با استفاده از نرم‌افزار SPSS 16 و با استفاده از آزمون‌هاي آماري توصيفي و تحليلي مورد تحليل قرار گرفت. ملاحظات اخلاقي: از كليه شركت‌كنندگان جهت شركت در مطالعه رضايت كتبي اخذ و نسبت به بي‌نام‌بودن پرسشنامه‌ها و محرمانه‌بودن اطلاعات اطمينان خاطر داده شد. يافته‌ها: بين تكرار درماندگي اخلاقي و ديدگاه پرستاران نسبت به رعايت حقوق بيمار ارتباط معكوس معني‌دار وجود داشت (P=0/04 ، r=-0/5). ميانگين نمره درماندگي اخلاقي در ابعاد شدت (0/7±3/97) و در ابعاد تكرار (0/68±3/60) بود. ميانگين ديدگاه پرستاران نسبت به رعايت حقوق بيمار (0/8±4) گزارش شد. ارتباط بين درماندگي اخلاقي با تعداد سال‌هاي خدمت و سن در پرستاران معني‌دار بود (0/05>P). بين نگرش پرستاران نسبت به رعايت حقوق بيماران با متغيرهاي جنس و تعداد سال‌هاي خدمت نيز ارتباط معني‌دار وجود داشت (0/05>P). نتيجه‌گيري: با توجه به اهميت درماندگي اخلاقي در پرستاران، مسؤولين بايستي ضمن حساسيت نسبت به ديدگاه پرستاران نسبت به رعايت حقوق بيمار، با استفاده از راه‌كارهايي ديدگاه پرستاران نسبت به رعايت حقوق بيماران را بهبود بخشند.

Background and Aim: Compliance with patients' rights is one of the most important components of providing humanitarian and moral care and nurses may be exposed to moral distress in defense of their rights. The present study was carried out aiming to determine the relationship between moral distress and nurses’ attitude towards patients' rights. Materials and Methods: The present correlation study was carried out on 194 nurses from educational hospitals affiliated to South Khorasan University of Medical Sciences who were selected with convenience sampling method and selected from critical care units. The data gathering tool was a demographic questionnaire and Corly's moral distress questionnaire and nurse’s attitude towards patients' rights researcher-made questionnaires. Collected data was analyzed by SPSS 16 using descriptive and analytical statistics. Ethical Considerations: Written consent was obtained from all participants. Additionally, all of them were assured of anonymity of the questionnaires and confidentiality of the information. Findings: Between moral distress frequency and attitude toward patients' rights, there have been significant positive correlation (r=-0.5, P=0.04). Average score of moral distress in the severity (3.97±0.7) and frequency dimensions (3.6±0.68). The average attitude towards the rights of patients (4±0.8) was also reported. The relationship between moral distress and the number of years of service and age in nurses was significant (P<0.05). There was a significant difference between nurses 'attitudes toward respect for patients' rights with gender and number of years of service (P<0.05). Conclusion: Regarding the importance of moral distress in nurses, authorities should be sensitive to the views of nurses regarding respect for patients' rights and using strategies to improve nurses' viewpoints towards respecting patients' rights and positive impact on ethical distress.
