چارچوبي براي ارزشيابي ارزيابان اعتباربخشي بيمارستان‌هاي ايران

A Conceptual Framework for Evaluation of Accreditation Surveyors of Iran Hospitals

مقدمه: اعتباربخشي به عنوان يك مدل شناخته شده براي ارزشيابي عملكرد مراكز درماني، با هدف ارتقاء كيفيت در نظام‌هاي سلامت كشورهاي مختلف به شمار مي‌رود كه موفقيت آن ارتباط مستقيمي با عملكرد مؤثر ارزيابان آن دارد. در همين راستا هدف اين مطالعه ارائه يك چارچوب مفهومي براي ارزشيابي ارزيابان اعتباربخشي بيمارستان‌هاي ايران مي‌باشد. روش بررسي: اين مطالعه كيفي با رويكرد پديدارشناسي در سال 1396 با بهره‌گيري از نظرات 30 نفر از افراد خبره و با تجربه در زمينه برنامه اعتباربخشي بيمارستان در سطوح وزارت بهداشت، دانشگاه‌هاي علوم پزشكي، بيمارستان‌ها و ارزيابان اعتباربخشي كه به روش نمونه‌گيري هدفمند با حداكثر تنوع انتخاب شده بودند، صورت گرفت. داده‌ها از طريق مصاحبه نيمه ساختار يافته جمع‌آوري و با روش تحليل محتوا تحليل شدند. يافته‌ها: چهار كد اصلي و بيست‌وهشت كد فرعي به دست آمدند. كدهاي اصلي شامل ضرورت و اهميت ارزشيابي ارزيابان، اصول و الزامات ارزشيابي ارزيابان، ساختار ارزشيابي ارزيابان و روش‌هاي ارزشيابي ارزيابان بودند. بحث و نتيجه‌گيري: نتايج اين پژوهش مي‌تواند چارچوب مؤثري براي ارزشيابي ارزيابان اعتباربخشي بيمارستان‌هاي كشور در اختيار متوليان نظام سلامت قرار دهد. به‌ عبارت‌ديگر ارزشيابي مستمر عملكرد ارزيابان اعتباربخشي با استفاده از روش‌هاي مختلف و مبتني بر اصول و الزامات ذكر شده در اين پژوهش مي‌تواند منجر به ارتقاء عملكرد ارزيابان اعتباربخشي و در نهايت تحقق بيشتر اهداف اعتباربخشي بيمارستان‌هاي كشور شود.

Background & Objectives: Accreditation, as a known method for evaluation of health care centers, is used with the aim of enhancing healthcare quality in the health systems of different countries and its success has a direct relation to the effective performance of its surveyors. So, the purpose of this study was to provide a conceptual framework for evaluating the accreditation surveyors of Iran hospitals. Methods: This qualitative study with a phenomenological approach was conducted in 2018 through eliciting the views of 30 expert and experienced interviewees in the field of hospital accreditation at the Ministry of Health, medical universities, hospitals, as well as accreditation surveyors selected purposefully with maximum diversity. Data were collected through semi-structured interviews and analyzed using content analysis. Results: A total of 4 main codes and 28 sub-codes were identified. Main codes included necessity and importance, principles and requirements, structure and methods of surveyors’ evaluation. Conclusion: The results of this study can provide an effective framework for health system administrators in evaluation of accreditation surveyors of Iranian hospitals. In other words, continuous evaluation of accreditation surveyors’ performance using different methods based on the principles and requirements found in this study can lead to the performance improvement of accreditation surveyors and finally the achievement of the accreditation goals.
