Comparison of the Quality of Physician-Nurse Relations in the Nurses in Two Groups of Acute Physiology and Chronic Health Evaluation-II and New Checklist of Health Status Assessment

زمينه و هدف : همكاري حرفه‌اي مناسب بين پرستاران و پزشكان، مي‌تواند در كيفيت مراقبت‌هاي بهداشتي ارايه شده به بيماران مؤثر باشد. مطالعه حاضر با هدف مقايسه تأثير دو سيستم ارزيابي فيزيولوژيك حاد و سلامت مزمن (Acute Physiology and Chronic Health Evaluation-II يا APACHE-II) و چك‌ليست جديد ارزيابي وضعيت سلامت بر كيفيت همكاري پزشك- پرستار انجام شد. مواد و روش‌ها: در اين پژوهش مقطعي- توصيفي- تحليلي، 100 نفر از پرستاران بخش مراقبت‌هاي ويژه بيمارستان امين اصفهان در سال 1396 شركت نمودند. جهت جمع‌آوري داده‌ها، از پرسش‌نامه نگرش نسبت به همكاري حرفه‌اي پزشك- پرستار Jefferson استفاده گرديد. در نهايت، داده‌ها با استفاده از آزمون‌هاي Paired t و Wilcoxon در نرم‌افزار SPSS مورد تجريه و تحليل قرار گرفت. 05/0 > P به عنوان سطح معني‌داري داده‌ها در نظر گرفته شد. يافته‌ها: ميانگين نمره رابطه پزشك- پرستار در گروه چك‌ليست جديد ارزيابي وضعيت سلامت به طور معني‌داري بيشتر از گروه APACHE-II بود (001/0 > P )، اما در مجموع، نگرش پرستاران نسبت به همكاري پزشك- پرستار در هر دو سيستم نمره‌دهي مثبت ارزيابي شد. نتيجه‌گيري: با توجه به نتايج به دست آمده، نگرش پرستاران نسبت به همكاري بين حرفه‌اي پزشك- پرستار در گروه چك‌ليست جديد ارزيابي وضعيت سلامت به طور معني‌داري مثبت‌تر از گروه APACHE-II مي‌باشد. بنابراين، استفاده از چك‌ليست جديد ارزيابي وضعيت سلامت به جاي سيستم APACHE-II در بخش مراقبت‌هاي ويژه بيمارستان‌هاي كشور توصيه مي‌شود.

Aim and Background: Appropriate professional collaboration between physicians and nurses can be effective on the quality of health care given to patients. This study was performed to compare the effects of Acute Physiology and Chronic Health Evaluation-II (APACHE-II) and new checklist of health status assessment on the quality of physician-nurse relationship. Methods and Materials: In this cross-sectional descriptive-analytical study, 100 nurses from intensive care unit (ICU) of Amin Hospital, Isfahan, Iran, participated. Jefferson Scale of Attitudes toward Physician-Nurse Collaboration was used for collecting data. Finally, data were analyzed using paired t-test and Wilcoxon test through SPSS software. P < 0.05 was considered statistically significant. Findings: The mean score of physicians-nurses relationship in new checklist of health status assessment group was significantly higher than that of APACHE-II group (P < 0.001). But overall, the attitudes toward physician-nurse collaboration were positive in both scoring systems. Conclusions: According to the results, attitudes of the nurses toward physician-nurse relationship in new checklist of health status assessment group was significantly more positive than APACHE-II group, so using new checklist of health status assessment instead of APACHE-II system is recommended in the ICUs of the country's hospitals.
