عوارض پوستي در اندام اهداكننده‌ي رگ و ارتباط آن با ديابت در افرادي كه عمل باي‌پس عروق كرونري انجام داده‌اند

Skin complications of blood vessel donor limbs and their association with diabetes in patients undergoing coronary artery bypass grafting

زمينه و هدف: با توجه به مشاهدات باليني و مطالعات محدود در زمينه‌ي عوارض پوستي و تغييرات رشد مو روي اندام اهداكننده‌ي رگ، اين مطالعه با هدف بررسي تغييرات رشد مو و عوارض پوستي روي اندام مربوطه در افرادي كه عمل باي‌پس عروق كرونري انجام داده‌اند و مقايسه‌ي ميزان تفاوت بروز اين عوارض در افراد ديابتي و غيرديابتي انجام شده است. روش اجرا: هر دو اندام بيماراني كه 6-3 ماه گذشته جراحي باي‌پس عروق كرونري داشتند و جهت پيگيري معمول به درمانگاه جراحي قلب مراجعه كرده بودند، بررسي و مقايسه شدند. تغييرات پوستي توسط متخصص پوست مورد بررسي قرار گرفته و پس از ثبت تغييرات به‌وجودآمده، بروز اين عوارض و تفاوت بروز در افراد ديابتي و غيرديابتي بررسي گرديد. يافته‌ها: از 97 بيمار كه وارد مطالعه شدند تعداد 77 نفر مرد و 20 نفر زن بودند كه از اين موارد 29 بيمار ديابت داشتند. طي بررسي‌ها 10 بيمار هايپرتريكوزيس، 3 بيمار ريزش مو در اندام مربوطه، 16 بيمار تغييرات هايپرپيگمانتاسيون، 21 بيمار خارش، 26 بيمار كلوييد، 3 بيمار علائم درماتيت، 9 بيمار عفونت زخم داشتند. در اين مطالعه هيچ كدام از عوارض در افراد ديابتي نسبت به افراد غيرديابتي تفاوت معناداري نداشت (P>0.05). نتيجه‌گيري: در اين مطالعه عوارض پوستي مشاهده‌شده مشابه ساير مطالعات انجام‌شده بود. تنها تفاوت بارز بروز هايپرتريكوزيس در اندام اهداكننده‌ي رگ بود. مطالعات بزرگتر با جمعيت بيشتر كه شامل پيگيري عوارض باشد لازم است تا علاوه‌بر تعيين دقيق‌تر شيوع عوارض، بهبود و پايداري آن‌ها را ارزيابي كند.

Background and Aim: According to the limited clinical observations and researches on cutaneous complications and hair growth changes of donor's site extremities, this study aimed to compare hair growth changes and cutaneous complications of donor's site extremity in patients who underwent coronary artery bypass graft between diabetic and non-diabetic patients. Methods: Patients who attended the heart surgical clinic for routine follow-up 3-6 months after CABG (coronary artery bypass grafting) were evaluated. Their donor's sites in both extremities were compared with each other. Cutaneous changes were assessed and recorded by a dermatologist. After recording data, the incidence of these complications and the differences between diabetic and non-diabetic patients were evaluated. Results: From the 97 participants of this study, 77 were male and 20 were female. Of these patients 29 were diabetic. Ten had hypertrichosis, 3 had hair loss in donor's site extremity, 16 had hyperpigmentation changes, 21 had itches, 26 had colloid scar changes, 3 had dermatitis, and 9 had wound site infection. In this study there was no statistically significant difference regarding the incidence of complications in diabetic patients compared to non-diabetic patients (P>0.05). Conclusion: The cutaneous complications in this study were similar to previous studies and the only significant finding was the incidence of hypertrichosis in donor's site extremity. In order to determine the prevalence and persistence of the complications more accurately, further studies on larger populations including follow-up of complications are warranted.
