مقايسه كاركردهاي اجرايي در بيماران مبتلا به اختلال شخصيت مرزي و دوقطبي II

Comparing the executive function in patients with borderline personality disorder and bipolar II disorder

مقدمه شباهت زياد ويژگي­هاي باليني اختلال شخصيت مرزي و دوقطبي-II علاوه بر مسائل تشخيصي، مجادلات علمي فراواني را درباره رابطه ميان اين دو اختلال به­ وجودآورده است. مطالعات مختلفي به بررسي شباهتها و تفاوتهاي اين دو اختلال در حوزههاي مختلف علامت شناسي، آسيب شناسي و سبب شناسي پرداختهاند. بر اين اساس هدف از مطالعه حاضر، بررسي و مقايسه كاركردهاي اجرايي در بيماران مبتلا به اختلال شخصيت مرزي و اختلال دوقطبي II به منظور شناخت بيشتر از رابطه ميان دو اختلال است. روش كار 25 بيمار مبتلا به اختلال شخصيت مرزي و 27 بيمار مبتلا به اختلال دوقطبي II ، با روش نمونه گيري در دسترس از بين بيماران مراجعه كننده به بيمارستان كامكار-عرب نياي شهر قم انتخاب شدند اين دو گروه با يكديگر و با 20 نفر از افراد سالم با استفاده از فرم كامپيوتري آزمون دسته­بندي كارتهاي ويسكانسينكه ابزاري براي ارزيابي كاركردهاي اجرايي است، مورد مقايسه قرار گرفتند. دادههاي بدست آمده با استفاده از روش­هاي آماري تي مستقل، خي دو و آنووا تجزيه و تحليل شدند. نتايج يافته­ هاي پژوهش نشان داد كه دو گروه باليني در تمامي كاركردهاي اجرايي عملكرد ضعيف­تري نسبت به گروه كنترل سالم داشتند. بيماران مبتلا به اختلال شخصيت مرزي ضعيف­ترين كاركرد را در حوزه انعطاف پذيري شناختي بدست آوردند. بيماران مبتلا به اختلال دوقطبي پايين ­ترين عملكرد را در ناتواني از دريافت بازخورد نشان دادند. بين دو گروه باليني تفاوت معناداري در هيچ يك از مولفه ­هاي كاركرد اجرايي مشاهده نشد. نتيجه ­گيري اين نتايج، حمايت تجربي در تاييد يافته­ هاي پيشين فراهم آورد كه حاكي از كاركردهاي اجرايي مختل در اين دو گروه از بيماران است.از سوي ديگر هرچند تفاوت معناداري بين دو گروه باليني در كاركردهاي اجرايي مشاهده نشد اما مطالعات گسترده براي روشن تر شدن رابطه ميان دو اختلال ضرورت دارد.

Introduction: Huge similarities between clinical characteristics of borderline personality disorder and bipolarII disorder has caused many conversations in terms of the relationships between two disorders. Many studies have investigated the similarities and differences of these two disorders in the field of symptomology, pathology and etiology. On this base, the goal of the current research was to investigate and compare the executive function of borderline and bipolar II disorders patients to know more about the relationship between these two disorders. Subjects & Methods: A total of 25 patients with borderline personality disorder and 27 patients with bipolar-II disorder, who were referred to the Kamkar-Arabnia hospital in Qom, were selected through convenience sampling method. These two groups were compared together and to a healthy control group, by Wisconsin Card Sorting Test, that is used to measure the executive functions. Collected data were analyzed by using independent t-test, X2 and ANOVA. Results: Results showed that two clinical groups had poorer performance in all of the executive functions in comparison with healthy controls. Patients with borderline personality disorder showed poorest performance on cognitive flexibility domain. Patients with bipolar II had lowest performance on inability to receive feedback. There was no significant difference between the two clinical groups in none of the executive functions domains. Conclusion: These results provides empirical support for previous findings, which have reported executive dysfunctions, in both groups of patients. Although, there is no significant difference between the two clinical groups in executive functions in this study, more extensive studies are needed for clarification of relationship between the disorders.
