بررسي وضعيت تجويز هورمون رشد در نسخ دفاتر درماني بيمه‌شدگان سازمان بيمه سلامت ايران در استان ايلام طي سال‌هاي 1396-1386

Investigation the Status of Prescribing Growth Hormone for Patients under the Coverage of the Iranian Health Organization in Ilam Province during 2007-2017

مقدمه: فرآيند رشد انسان از بدو تشكيل جنين شروع و به مرگ خاتمه مي‌يابد. هورمون رشد هورموني است كه از هيپوفيز ترشح و در فرآيند رشد دخالت دارد. 11 تا 28 درصد علل كوتاهي قد در ايران و حدود 10 درصد در جهان به كمبود هورمون رشد اختصاص دارد. در اين مقاله وضعيت تجويز هورمون رشد براي بيمه‌شدگان بيمه سلامت در سال‌هاي 1396-1386 در استان ايلام مورد بررسي قرار مي‌گيرد. روش بررسي: مطالعه از نوع مقطعي توصيفي و گذشته نگر است و با استناد به اسناد و مدارك موجود در اداره كل بيمه سلامت انجام شده است. جامعه آماري مطالعه شامل كليه نسخ دارويي بيمه‌شدگان سازمان بيمه سلامت حاوي هورمون رشد در سال‌هاي 1396-1387 مي‌باشد كه توسط داروخانه‌ها به اداره كل بيمه سلامت استان ايلام ارسال شده است. با استفاده از سوابق موجود در بانك اطلاعات و پردازشگر بيمه سلامت، و به روش سرشماري، توصيف داده‌ها از طريق نرم‌افزارهاي Excel ويرايش 2007 و SPSS ويرايش 21 صورت گرفت و نتايج در قالب جداول فراواني و نمودار ارايه شد. يافته‌ها: يافته‌ها نشان داد كه تعداد هورمون رشد تجويز شده از سال 1386 تا سال 1396 روند افزايشي داشته و از 1858 عدد به 4485 عدد رسيده است كه روند افزايشي معادل 82/2 برابر است. همين طور تعداد نسخ تجويز شده در سال 1386 جمعاً 66 نسخه بوده كه با رشد 78/7 برابري به 513 نسخه افزايش يافته است. ميانگين هزينه هر نسخه حاوي دارو هورمون رشد در استان ايلام از مبلغ 606/399/3 ريال در سال 1386 به مبلغ 125/972/14 ريال در سال 1396 رسيده است. نتيجه‌گيري: هورمون رشد يكي از اقلام دارويي هزينه‌ساز در سازمان بيمه سلامت است اين دارو در سال‌هاي مورد مطالعه 1396- 1386 همواره در رتبه‌هاي بالا قرار داشته و سهم قابل ملاحظه‌اي از هزينه‌هاي دارويي بيمه سلامت استان ايلام را به خود اختصاص داده است و در تمام اين سال‌ها جزو 20 قلم داروي هزينه‌ساز استان بوده است و با گذشت زمان رتبه هزينه‌اي آن افزايش يافته و در دو سال 96- 1395 به جايگاه اول رسيده است. كنترل و نظارت بيشتر بر نحوه تجويز و مصرف آن در بهينه نمودن هزينه هاي دارويي اداره كل بيمه سلامت استان ايلام مؤثر خواهد بود.

Introduction: Human growth begins since the formation of the embryo and ends with death. Growth hormone is secreted from the pituitary gland and involved in the growth process. 11 to 28 percent of short stature in Iran and about 10 percent in the world is related to growth hormone deficiency. In this article, the status of prescribing growth hormone for those under the coverage of Iranian Health Insurance was investigated in Ilam province during 2007-2017. Methods: This was a cross-sectional descriptive and retrospective study and conducted according to the available documents in the General Directorate of Iranian Health Insurance. The statistical population of the study included all medical prescriptions of growth hormone to those under the coverage of the Iranian Health Insurance during 2007-2017 sent by pharmacies to the general directorate of Ilam Health Insurance. Using the records in the database and the health insurance processor, and by census method, the data entered Excel 2007 and analyzed using SPSS version 21. Results: The number of prescribed hormone in 2007 was 1858 which increased to 4485 in 2017, showing a 2.82 folds increase. Also, the number of prescriptions in 2007 was 66 totally, which increased by 78.7 times to 513 prescriptions. The average cost of each version containing the growth hormone in Ilam province was 3,699.606 Rials in 2007 that increased to 14.972.125 Rials in 2017. Conclusions: Growth hormone is one of the expensive drugs in the health insurance organization. The drug in the studied years of 2007-2017 has always been at the top ranks with a significant role in medical expenses of Ilam health insurance, and has been one of the top 20 costing items in the province. Also, over time, its cost ranking increased and reached to the first in 2016-2017. More control and supervision on how to prescribe would be effective in optimizing the pharmaceutical costs of the general directorate of health insurance of Ilam province.
