سابقه و هدف: اهميت رعايت اصول استاندارد در انجام ساكشن باز لوله داخل تراشه و همچنين آگاهي از عوارض اين نوع ساكشن همواره به عنوان يكي از چالش هاي مراقبت از بيماران در بخش هاي مراقبت ويژه بوده است. هدف از اين مطالعه تعيين ارتباط نمره درد با رعايت استانداردها در ساكشن باز اندوتراكئال نوزادان تحت ونتيلاتور در بخش مراقبت هاي ويژه نوزادان بيمارستان هاي شهر همدان بود.
مواد و روش‌‌ها: مطالعه حاضر توصيفي از نوع همبستگي بود كه در بخش مراقبت هاي ويژه نوزادان بيمارستان هاي همدان بر روي 120 نوزاد نارس تحت ونتيلاتور انجام شد. پژوهشگر حين و بعد از ساكشن باز اندوتراكئال نوزاد توسط پرستار، انجام درست پروسيجرهاي شستن دست ها، پوشيدن دستكش، تعداد دفعات ساكشن، مدت زمان ساكشن كردن، سايز كاتتر مورد نياز براي ساكشن كردن و كنترل فشار دستگاه ساكشن توسط پرستار مربوطه را ثبت نمود. داده ها با استفاده از آزمون هاي توصيفي و رگرسيون مورد تجزيه و تحليل قرار گرفتند.
يافته‌ها: بر اساس نتايج مدل تطبيق يافته رگرسيون خطي بين فشار دستگاه ساكشن، مدت زمان ساكشن و تعداد دفعات ساكشن با نمره درد ارتباط معني دار آماري وجود داشت (0/000P=) اما بين سايز كاتتر ساكشن با نمره درد ارتباط معني دار آماري وجود نداشت (0/235P=).
نتيجه‌گيري: رعايت استانداردهاي ساكشن باز اندوتراكئال در نوزادان نارس مي تواند به ميزان بالايي به ثبات شاخص هاي فيزيولوژيك، تسريع روند بهبودي و برگشت سريع نوزادان كمك كرده و از بروز عوارض و افزايش طول مدت بستري جلوگيري نمايد
چكيده لاتين :
Background and Objective: Although suction is an important nursing intervention in neonates under ventilation, it can lead to complications such as pain, respiratory distress, hypoxemia, bronchospasm and hospital infection. The purpose of this study was to determine the relationship between pain score and its effective indices in neonatal endotracheal open suction under ventilator in the neonatal intensive care unit of Hamadan hospitals.
Materials and Methods: This descriptive correlational study was performed on 120 preterm infants under ventilation in Neonatal intensive care unit of Hamadan hospitals. First, 5 minutes before the patient suction, the respiratory rate and oxygen saturation of the neonatal arterial blood were recorded by the researcher in three shifts based on the findings of the direct monitoring of the infant. During and 5 minutes after suction, the respiratory rate and oxygen saturation index was re-recorded. Data were analyzed using descriptive and regression tests.
Results: According on the results of the applied linear regression model, there was a statistically significant relationship between the pressure of the suction device, the duration of suction and the number of suctions with the pain score(P =0.000) but there was no statistically significant relationship between the size of the suction catheter and the pain score (P =0.235).
Conclusion: Endotracheal open suction in premature infants can greatly affect the number of respiration and oxygen saturation in the arterial blood and increase the pain score in these infants.
