طراحي الگوي ارتقاي كيفيت آموزش دستياري در اجراي طرح تحول نظام سلامت

Designing a model to improve the quality of residents’ education in implementing the Health System Transformation Plan

با توجه به ادغام آموزش و درمان در بيمارستان­ هاي آموزشي ، كيفيت آموزش دستياري از اجراي طرح تحول نظام سلامت تاثير پذيرفته است. هدف از اين مطالعه طراحي الگوي ارتقاي كيفيت آموزش دستياري براساس اجراي طرح تحول نظام سلامت بود. روش كار: مطالعه كيفي حاضر با نظريه داده بنياد در بهار 1398 بر روي 24 نفر از مديران، استادان و دستياران دانشگاه علوم پزشكي مشهد به روش نمونه­ گيري هدفمند و تا رسيدن به اشباع نظري و با رعايت محرمانه­ بودن اطلاعات انجام شد. داده ­ها به روش مصاحبه نيمه ساختار يافته جمع­ آوري و با نرم ­افزار MAXQDA10 كدگذاري باز انجام شد در مرحله بعد با دسته ­بندي كدهاي باز مقوله­ هاي محوري مشخص و در مرحله آخر با مشخص شدن ارتباط بين مقوله­ هاي محوري (كدگذاري گزينشي) انجام شد. يافته­ ها:نتايج تحليل داده­ هاي قالب الگو عبارتست از: شرايط زمينه ­اي (بار درمان)، شرايط علي (آموزش باليني) و شرايط مداخله­ گر (نظارت بر فرآيند)، راهبردها (استانداردسازي آموزش، كيفي ­سازي آموزش، اعتلاي اخلاق حرفه­ اي و حفظ كرامت انساني و بررسي همه جانبه طرح تحول قبل از اجرا) و پيامدهاي ارتقاي كيفيت آموزش دستياري (كاهش كار درماني استاد و دستيار، ايجاد انگيزه در دستياران، كاهش خطاي پزشكي، افزايش زمان آموزش دستياران، افزايش نظارت بر كار درماني استاد و دستيار) بود. نتيجه ­گيري:افزايش كيفيت و كميت آموزش در دانشگاه ­هاي علوم­پزشكي قطعاً به مرور زمان در نحوه درمان بيماران و در سلامت افراد تأثير مثبتي خواهد داشت. نتايج تحقيق نشان داد با ايجاد زيرساخت ­هاي مناسب در بيمارستان­ هاي آموزشي، قبل و بعد از اجراي طرح تحول نظام سلامت، كيفيت آموزش دستياران به عنوان يكي اركان­ هاي كليدي در ارتقاء و بهبود عملكرد سيستم آموزشي و درماني ارتقاء پيدا خواهد كرد .

Due to the integration of education and treatment in teaching hospitals, the quality of residents’ education has been affected by the implementation of the healthcare transformation plan system. The purpose of this study was to provide a model for improving the quality of Residents Education based on healthcare transformation plan. Methods: This qualitative study was conducted on 24 managers, professors and assistants of Mashhad University of Medical Sciences using a grounded theory approach and Purposeful sampling was performed until theoretical saturation and confidentiality of information was achieved. Data were collected through semi-structured interviews and open coding was done using MAXQDA10, in the next step the open source categories were identified as axial categories, and in the last stage, the relationship between the axial categories (selective coding) was determined. Results: Findings based on analyzing data including : ground condition (burden of treatment), causal condition (clinical education), intervening condition (supervision process) and strategies (education standardization, Qualification of education, promotion of professional ethics and preservation of human dignity and comprehensive review of pre-implementation transformation plan), and implications (reducing teacher and assistant therapeutic work, motivating residents, reducing medical error, increasing education time ,increasing supervision of occupational therapist and assistant). Conclusion: Increasing the quality and quantity of education in medical universities will definitely have a positive effect on how patients are treated and on their health over time. The results showed that by creating appropriate infrastructure in educational hospitals, quality of residentschr('39') education will be improved as one of the key elements in promoting and improving educational system performance before and after implementation of health system development plan.
