بررسي عملكرد پرستاران در كاربرد اصول اخلاقي حين انجام پانسمان بيماران در بيمارستان شهيد بهشتي كاشان، سال 1396

The Assessment of the Nurses' Performance in Applying Ethical Principle While Bandaging Wounds in Shahid Beheshti Hospital of Kashan, Iran, in 2016

سابقه و هدف: رعايت اخلاق لازمه مراقبت‌هاي باليني است. يكي از مراقبت‌هاي پرستاري، مراقبت از زخم و انجام پانسمان است. در مراقبت‌هاي پرستاري ازجمله پانسمان، علاوه بر رعايت روش صحيح، رعايت اصول اخلاقي نيز ضروري است. بااين‌وجود در نتايج برخي از مطالعات، عملكرد پرستاران در اين زمينه رضايت‌بخش نبوده است. با توجه به اهميت اين موضوع، پژوهش حاضر با هدف تعيين عملكرد پرستاران در كاربرد اصول اخلاقي در پانسمان انجام شد. مواد و روش‌ها: مطالعه حاضر به روش توصيفي-مقطعي از ارديبهشت 1396 تا آذر همان سال روي 200 پرستار بيمارستان شهيد بهشتي كاشان انجام شد. روش نمونه‌گيري به‌صورت آسان بود و داده‌ها از طريق چك‌ليست محقق‌ساخته جمع‌آوري شد كه شامل اطلاعات دموگرافيك و بررسي عملكرد پرستاران در رعايت اصول اخلاق در پانسمان بود. داده‌ها از طريق آزمون‌هاي يو من‌ويتني و همبستگي پيرسون تحليل شدند و سطح معني‌داري كمتر از 0/05 در نظر گرفته شد. يافته‌ها: نتايج نشان داد ميانگين سني پرستاران 6/52 ± 24/87 بود. درمجموع 66 درصد از نمونه‌ها مؤنث، 61/5 درصد مجرد و از نظر وضعيت شغلي، 75 درصد از مشمولين طرح نيروي انساني بودند. ميانگين نمره كسب‌شده از چك‌ليست با دامنه نمره 0 تا 30 برابر 4/11 ± 20/63 بود. 57 درصد از عملكرد پرستاران در كاربرد اصول اخلاقي حين پانسمان بيماران در سطح خوب قرار داشت. از بين متغيرهاي زمينه‌اي، وضعيت تأهل و وضعيت شغلي با نمره كل رابطه معني‌داري نشان ‌داد (0/05>P)، به‌طوري‌كه ميانگين نمره پرستاران متأهل و مشمولين طرح نيروي انساني از ديگر پرستاران بيشتر بود. استنتاج: نتايج حاكي از آن بود كه تنها نيمي از پرستاران عملكرد خوبي داشته‌اند و ميانگين نمره آن‌ها در سطح متوسط قرار داشته است. ازاين‌رو پيشنهاد مي‌شود مسئولان برنامه‌ريزي و اقدامات لازم را براي شناسايي علل و عوامل بروز كاستي‌ها انجام دهند و خط‌مشي‌هاي مربوط به اين موارد را در نظر بگيرند.

Background and Purpose: Adherence to ethics is essential for clinical care. One of the nursing cares is wound care and dressing. Nurses should perform dressing by the adoption of the correct technique, along with the observation of ethical principles. However, some previous studies have shown that the performance of some nurses in this regard was not satisfactory. Therefore, due to the importance of this issue, this study aimed to determine the performance of nurses in adherence to ethical principles in dressing. Materials and Methods: This descriptive cross-sectional study was conducted on 200 nurses of Shahid Beheshti Hospital in Kashan, Iran, during 8 months in 2016 using convenience sampling. The data were collected using a researcher-made checklist comprising demographic information and performance of nurses in adherence to ethical principles in dressing. The data were analyzed using the Mann-Whitney U test and Pearson correlation. A p-value of less than 0.05 was considered statistically significant. Results: The mean age of the nurses was 24.87±6.52 years, and 66% of the study subjects were female. In addition, 61.5% of the participants were single and 75% of them served Manpower Planning. The obtained mean score of the questionnaire within the range of 0-30 was reported as 20.63±4.11. Furthermore, 57% of the nurses applied ethical principles well while dressing patients. Among the underlying variables, there was a significant relationship between marital status and occupational status with the total score (P<0.05). Conclusion: The results showed that only half of the nurses performed well and their mean score was at a moderate level. Therefore, it is suggested that authorities plan and take necessary measures to identify the causes of shortcomings and take the policies associated with these ethical issues into account.
